An Engagement Plan captures significant details, events, and memories from a person’s life in a personalized narrative. It helps caregivers, family members, and healthcare professionals understand and connect with an individual on a deeper level.

By creating an Engagement Plan, we improve the quality of care, making it more person-centered. Personal information enhances communication, providing comfort and a sense of identity for individuals with dementia.

Creating Personalized Engagement Plans Through Life Stories

Engagement Plans are the perfect opportunity for families and staff to become familiar with the resident’s life story. This helps us create a plan tailored to the individual, ensuring they engage daily while encouraging them to remain independent and live life to the fullest. Our plans aim to provide each resident with a person-centered base of care that meets their specific needs.

To prepare for your loved one’s future care, always note their likes and dislikes. Ask them about their childhood activities, favorite TV shows, and preferred vacation spots. The more detailed you are about your loved one’s past, the more beneficial our Engagement Plans will be for our staff.
