Early Detection: A Team-Based Approach

Category: Health & Wellness

Author: Nicole Hensley, Wellness Director

Early detection is essential to keeping residents at The Princeton Senior Living safe and well. Subtle shifts in behavior, appetite, mobility, or mood often signal that something may be wrong. When we recognize concerns early, we intervene quickly and reduce the risk of serious health complications. This proactive approach protects residents and supports better long-term outcomes.

Effective early detection requires consistent observation, clear communication, and the confidence to act without hesitation.

Building a Culture of Accountability

I encourage staff to report even the smallest change in a resident. At The Princeton, we live out a “see something, say something” culture across all departments. I regularly remind team members that a small change is still a change.

I rely on staff to serve as my eyes and ears throughout the community. Because they interact with residents daily, they often notice concerns first. When team members share observations promptly, we protect resident safety and strengthen our team approach to care.

Daily Practices That Support Early Detection

Stand-up meetings and community rounding play a vital role in early detection. These structured routines create regular opportunities to share updates and concerns. Purposeful rounding allows staff to observe residents closely and recognize subtle changes before they escalate.

We also conduct two-hour post-fall checks to monitor residents carefully after an incident. This practice helps us track progress, identify new symptoms, and respond quickly if needed. By staying consistent with these routines, we reduce risk and improve follow-up care.

Strengthening Family Confidence

When we identify and address changes early, families feel informed and supported. Clear communication allows them to ask questions and participate in care decisions. This transparency builds trust and reinforces confidence in the care their loved one receives.

Early detection also helps residents remain safely in their apartments longer. By addressing concerns promptly, we prevent avoidable complications and reduce unnecessary transitions. At the same time, we give families peace of mind knowing their loved one is closely monitored.

When we combine early recognition with timely assessment and thorough documentation, our team responds with clarity and purpose. Through this proactive approach, we continue delivering thoughtful, person-centered care that helps residents thrive at The Princeton Senior Living.
